How to apply for a Credential Document Review (CDR) Letter from SSMLT?

This is not a membership and receiving a positive CDR letter does not guarantee membership with SSMLT in the future. This process is for MLTs that are not currently in Canada or are in Canada but not currently eligible to work in Canada.
SSMLT will review credentials and currency of practice documents to determine if you meet the initial requirements to become a member. 
The letter may be used as part of your application for Saskatchewan Immigrant Nominee Program (SINP).
Should you arrive in Canada and become eligible to work in Canada you must apply to become a Practicing or Temporary Practicing-working under supervision member and meet all requirements outlined above.

1.  You must first have completed a Prior Learning Assessment (PLA) conducted by the Canadian Society for Medical Laboratory Science (CSMLS) and be deemed eligible to write the CSMLS certification exam.  Following that you must request CSMLS send to SSMLT a completed PLA report that indicates you are eligible to write the CSMLS certification exam.  If you have not already completed this step, review Prior Learning Assessment (PLA) information on the CSMLS website.  Any questions related to PLA process should be directed to CSMLS.
2.  Received certification by successfully completing the certification exam from CSMLS; or 
     Received statement of eligibility to write the certification exam from CSMLS
         -review sections 5 and 14 of the SSMLT Regulatory Bylaws for terms and limits.   
3.  Provide evidence of Currency of MLT Practice:
         -Submit required Practice Hours
         -Submit required Continuing Professional Education

 

Document Checklist

Please note that once you submit your on-line credential document review form, your credential documents review application will have a status of 'pending'. When all required credential documents are received, SSMLT staff will review your application and send you a CDR letter to advise you as follows:

  • You meet the initial requirements for SSMLT membership; or
  • You are required to submit further documents; or
  • You that you do not meet the initial requirements for SSMLT membership

    CDR Term:
    Credential document review file expires 12 months from the date of initial application.
    Should you meet the requirements for SSMLT license application, you must proceed to license application prior to file expiration.
    If you do not apply for a license prior to file expiration, you must submit a new Credential Document Review application, required updated documents and applicable fee.

    Eligibility for membership/license following arrival in Canada you must:

    Meet the general requirements for membership:
             - language requirement
             - proof of good character
             - submit photo identification

    How to Apply

    Once you have reviewed the applicable document checklist and have all required documents you can complete the Credential Document Review Application Form below, remit the fee (non-refundable $150) and submit required documents.

    Some documents are required to be sent from source directly to SSMLT office as outlined in the document checklist. Application processing will not begin until all documents are uploaded or received from source and takes up to 3 weeks.

    Requirements

    • Enter all required profile information.
    • Pay applicable credential document review fees.
    • Upload your supporting documents through your "Applicant Portal"

      The on-line application will allow you to create your own login-id and password.

      Username requires a minimum length of 6 characters.

      Password requires a minimum length of 8 characters, must contain an uppercase letter, a lower case letter, and a digit.

      These login credentials which you may reset, will allow you access to your 'Applicant Portal' where you can upload the required supporting documents.
