Acknowledgement of Supervision
In order to fulfill the specific requirements for temporary licensing with the Saskatchewan Society of Medical Laboratory Technologists, a member is required to work, at all times, under the direct supervision of a fully licensed MLT.
The form below is required to be completed in full and submitted to SSMLT by all members with a Temporary Licence.
